He is survived by: his parents, Jimmy Dale and Shirley Fay Collins Lewis; his wife Alicia Lynn Reed Lewis; his daughters, Haley Brook Lewis of Salyersville and Lindzie Breann Hensley (Steve) of West Liberty; his sister Cheryl Lynn Phillips (Brian) of West Liberty; and his granddaughter Harper Blaire Lewis.
A funeral service was held on Monday, July 28th 2025 at 1:00 PM at the White Oak Church of God (6101 Grape Creek Rd, West Liberty, KY 41465).
