He was predeceased by: his grandson Nicholas A. Prokopchak; and his parents, Michael J Prokopchak and Edna M. Prokopchak. He is survived by: his children, Lesa M. Prokopchak-Peters (Scott) of Galveston, TX, Leonard E. Prokopchak (Kimberly) of Williamstown, KY and Lori L. Prokopchak-Hornsby (Bill) of Lexington; his sibling Michele E; and his former wife Cheryl L. Prokopchak of Crittenden, KY.. He is also survived by five grandchildren; 17 great grandchildren; one great -great grandchild.
