He is survived by: his wife Tina Washburn Polk of Willisburg; his daughter Jaclyn Polk (David Burns) of Willisburg; his mother-in-law Jean Washburn of Willisburg; his sister-in-law Jeanette McCue of Bardstown; his nephew Josh McCue (Mia Soloria) of Lebanon; his great-nephews, Harley and Sawyer; and his brother Steve Miller (Lorie) of Taylorsville.
