He was predeceased by: his parent Willie B. Perkins (Law); his wife Tammy Mitchel; his daughters, Cara Skirvin and Cassie Slusher; and his siblings, Shupie Perkins and Elizabeth West.
He is survived by: his siblings, Barbara Wilson of Carrollton, Ruby Anderson (Charlie) of Campbellsburg, Kentucky and John Perkins (Babara) of Worthville; and his father Clarence Wallace, Jr. of Bedford.
