He was predeceased by: his wife Dorothy Jean Cook; his parents, James Ezra and Edna Earl Cook; and his siblings, Maxine Gates, Virginia Wells, Pelza Cook, Horace Cook, Ray Cook, Glenn Cook and Marion Cook.
He is survived by: his sons, Kenneth Wayne "Ken" Cook, Jr. (Annie) and David Keith Cook (Phyllis); his sisters, Imogene Ralph and Mary Hogan; his grandchildren, Elizabeth Ann Hoskins (Jason), Ezra Keith Cook (Heather), Kenneth Wayne "Bo" Cook III (Kelly), Luke Harrison Cook and Lindsey Erin Morris (Brandon); and his great grandchildren, Micah Aaron Morris, Hannah Rae Cook, Mackenzie Jean Cook, Lily Elizabeth Hoskins, Chloe Elizabeth Cook, John Mark Cook, Allie Belle Morris, Ella Kate Morris and Ava Rose Morris.
