He is survived by: his mother Rhonda Church; his sons, Carter Gregory and Micah Alexander Church; his grandparents, Hazel Carter, Carl and Jo Ann Church; and his girlfriend Mandy Cater. He is also survived by numerous aunts, uncles, cousins and friends.
