He was predeceased by: his parents, Calvin Cook and Gracie Cook; his daughter Clara Jacks; and his siblings, Talmadge Walthall, Toney Walthall, Carl Walthall, Jr., Floyd Cook, Lloyd Cook, Ivy Cook, Harold Cook, Alline Bonds, Virginia Plumbley, Calvin Cook, Jr., J. P. Cook, Frank Cook and Gracie Bell Cook.
He is survived by: his wife Marcia Cook; his children, Laura Willett (Terry) of Alexandria, LA, Tina Reynolds (Mitch) of Bastrop, LA, Ann Cook of Waxahachie, TX, Bridgett Carpenter (Jamie) of Bastrop, LA, James Cook (Tiffany) of Alexandria, LA and Shannon Sarver (Tammy) of Montgomery, LA; and his brother David Arlan Cook (Dollie) of Ft. Worth, TX. He is also survived by numerous nieces, nephews.
In lieu of flowers, memorial donations may be made to Tunnel 2 Towers or St. Jude's Childrens Hospital.
