He was predeceased by: his parents, Charles, Sr. Middlebrooks and JoAnn Miller Middlebrooks; and his brothers, Charles Ray Middlebrooks, Jr. and Alan Roy Middlebrooks. He is survived by: his sisters, Christine Prather (Barry) of Eunice, Belinda Foret (Leroy) of Plaucheville and Rosalind Ann Middlebrooks of Marksville. He is also survived by many nieces and nephews and great nieces and great nephews.
