He was predeceased by: his son Ronnie Cook; his parents, Pete Cook and Della Mae Snell Cook; and his sisters, Lynette Frith (Robert) and Connie Cook.
He is survived by: his wife Rachel Cook; his children, Jamie Cook (Mitchell McKinnie), Carrie Watson and Donnie Cook; and his sisters, Louise Cook and Linda Cook.
