He was predeceased by: his granddaughter Zyla Celeste P.; his parents, Frances Polk and Elmer "Jay" Polk Jr.; and his brother Michael A. Polk. He is survived by: his children, Jenni Polk, Jozie LeCompte (Sam), Jeremy Polk (Megan) and Summer Volquardts (Dodi); his granddaughters, Adell, Addison, Abree, Alexis, Alanis, Ava, Ariah and Mya; and his siblings, Teri Ellsworth and Shane Polk (Gayla).
