She was predeceased by : her husband Norman "Craig" Forrest; her father W. C. Cook; and her sister-in-law Kathy Dale Forrest. She is survived by : her children, Trent Forrest (Macy) and Traci Forrest Jennings (Ryan); her grandchildren, Connor, Kale Forrest, Parker and Peyton Jennings; her mother Elaine Cook; and her siblings, Tam Brooks (Luke), Brad Cook (Leslie) and Randy Cook (Dawn). She is also survived by numerous nieces and nephews.
In lieu of flowers, the family has requested that donations be made to the Richard Murphy Hospice House (http://www.richardmurphyhospice.com) or the SLU Lady Lions PRIDE program, (SLU Women's Basketball PRIDE, SLU Box 10309, Hammond, LA 70402).
