He was predeceased by: his wife Patsy Jack; his parents, Leroy Jack, Sr. and Mary Francis Jack; his stepfather Leroy Benjamin; his siblings, Cheryl Edwards, Marie Jack, Clayton Francis, Sr., Alton Jack, John Francis, Anthony Jack, Sr. and Leroy Jack, Jr.; and his great-nephew Jaylan "Pop" Ben.

He is survived by: his siblings, Rusty Jack, Sr. (Tessie), Barbara Celestine and Michael Jack; and his nephews, Leo "Yola" Jack, Thyrome "Sudie" Jack and Warren Jack. He is also survived by nephews, nieces.
