He was predeceased by: his parents, Robert Cook and Betty Bruner Cook; and his brother Brian Cook. He is survived by: his wife Darlene Cook; his children, Tatum Cook of Cecilia and Kaleigh Cook of Lafayette; his grandchildren, Simon Huval and Odette Huval; his brother Steve Cook (Risë Koben) of Bloomington, Indiana; and his nieces and nephews, Kelly Cook of Atlanta, Georgia and Andrew Daichi Cook of Tokyo, Japan..

In lieu of flowers, the family ask that donations be made to Michael J. Fox Foundation for Parkinson's Research in memory of Kevin.
