Kathy Jean York Forman Obituary

It is with great sadness that we announce the death of Kathy Jean York Forman of Monterey, Louisiana, who passed away on May 26, 2026, at the age of 75, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was predeceased by: her parents, Howard "Pete" York and Beatrice Regan York; her grandparents, Robert York, Maggie Spann York, Cornelius Regan and LeLenre Regan; and her parents-in-law, Honeycutt and Louise Forman.

She is survived by: her husband Wesley Forman, Jr. of Monterey, LA; her sons, Wesley Forman, III (Ragenia) of Clayton, LA and Kris Forman (Lisette) of Vidalia, LA; her siblings, Joycie Gentry of Jonesville, LA, Joan St. Clergy (Ralph) of Marksville, LA and Neil York of Jonesville, LA; her grandchildren, Mary Cirilo (Curtis) of Clayton, LA, Ashley Bonsall (Ethan) of Vidalia, LA, Ryan Forman (Lauren) of Vidalia, LA and Annalee; her great grandchildren, Aubrey, Aleigha, Adeline Cirilo, Remi Bonsall, Luke, Josie Claire Forman and Elliott; and her cousin Brayden. She is also survived by her grandchildren and great-grandchildren.
