She was predeceased by: her husband Michael J. Ellsworth; her parents, Earl Myers and Betty J. Smith; her son Theodore McCullum, Jr.; her stepfather Charles Dickerson Smith; and her grandson Kar'Nas Coleman.
She is survived by: her children, Aquanilla Smith, Theota Smith (Irian) and Kentron Smith; her siblings, Mary Smith, Denise Parks (Milton) and Edward Washington (Linda); her aunt Helen Valtine; and her uncle Paul Toney (Trish). She is also survived by 18 grandchildren, 4 great-grandchildren, and a host of nieces, nephews, cousins, family and friends.
