He is survived by: his wife Nancy Cook (Book); his children, Jennifer Cook of Searcy, AR, Joseph Cook of Providence, RI, Joanna Cook of Boston, MA and John Cook (Shaysa) of Madison, WI; his siblings, Tracey Western of Searcy, AR and Tipton Cook (Kate) of Bailey, CO; his stepchildren, Tyler Fross (Hayley), Hayley (Fross) and Gottschalk (Derek); his stepgrandchildren, Skylar, Leigha, Lily, Oliver, Clark and Elijah; and his nieces and nephews, Paul, Luke, Canyon, Rosie, Elijah, Corey and Aaron.
