He was predeceased by: his parents, George Forar and Donna Forar. He is survived by: his wife Janis Forar; his children, Treasa Forar, Monica Forar (Brandon) and Gina Zelden (Matt); his grandchildren, Riley, Kira, Cameron Chappetta, Gabriella Forar, Laila Taberski, Sky, Bart, Joe, Ladonna, Luna, Scarlett, Zayne Sample, Troy, David and Avery Zelden; and his sister Debra Forar. He is also survived by many extended family members and friends.
