He was predeceased by : his parents, Samuel Polk, Sr. and Althea Clark Polk; his grandparents, Cleveland, Roberta Clark, Simuel Polk and Ollie M. Daniels; and his siblings, Samuel Polk, Sr., Cleveland Polk, Annie Polk Roby, Carole Polk Alexander, Alfreda Polk Bruster and Jacqueline Polk.

He is survived by : his children, Michelle Polk Tobias (Pernell), Calvin Wesley Polk, Jr. (Stacy), Demond Curtis Polk, Sr. (Yolanda), Ireannna Shauntrice Veal and Rene Green (Johnathan); and his siblings, Freddie Polk, Stanley Polk, Sr., Daisy Polk Woods, Donna Polk, Denise Ramee and Antoine Smith. He is also survived by 16 grandchildren, 6 great grandchildren, and a host of nieces, nephews, cousins, other relatives, and friends.
