He was predeceased by: his grandparents, T. C. and Nell Lewis. He is survived by: his parents, Tom (Michele Lewis) and Kevin (Jodi Cook); his grandparents, Joel, Deanna Cook, John and Joann Duckworth; and his siblings, Joshua Cook (Kimberly), Matthew Lewis, Madison Lewis, Thomas Lewis Jr., Jessica Cook and Kaden Cook (Alley). He is also survived by numerous aunts, uncles, nieces, nephews, and other extended family.
