He was predeceased by: his parents, Randall Middlebrook and Marguerite Middlebrook (Jenks). He is survived by: his wife Joan I. Lemoine; his children, Timothy Middlebrook (Cynthia) of Summerville, SC and Kimberley Dubis of Adams; and his grandchildren, Ethan, Griffin Dubis, Jacob and Norah Middlebrook. He is also survived by nieces and nephews.
