She was predeceased by: her parents, Arthur Eisman and Edna Montos (Bien); her husband James Formaggioni; her grandson Jimmy Formaggioni; and her brother-in-law Art Bernier, II.
She is survived by: her sons, Robert Formaggioni (Beth) and David Formaggioni; her sister Carol Bernier; and her sibling's children, Carol DeLevo, Cindy Laflamme and Art Bernier, III.
Visitation will be held on Tuesday, December 9th 2025 from 9:30 AM to 11:00 AM at the Colonial Forastiere Funeral & Cremation (985 Main St, Agawam, MA 01001). A funeral service will be held on Tuesday, December 9th 2025 at 11:00 AM and at 12:00 PM.
