He was predeceased by: his parents, John T. McCormack, Sr. and Evelyn McCormack (Butcher); his children, Dolores "Dee" McCormack and John "Jack" T. McCormack, III; and his siblings, Malcolm McCormack, Evelyn Grace Roca and Wayne E. McCormack.
He is survived by: his wife Anne "Anna" Marie McCormack (Restino); his children, Douglas McCormack of North Attleboro, MA and Sandra Kogut (Kevin Kogut) of Attleboro, MA; his grandchildren, Douglas, Steven, Blake, Vinny and Erica; his great grandchildren, Vincent, Jonathan, Leon, Lukas and Lainey; and his sister Emily Mosher of Attleboro, MA. He is also survived by several extended family members and dear friends.
