He was predeceased by: his parents, H. Calvin Cook and Ruth Cook (Transue); and his brother H. Calvin Cook, Jr.. He is survived by: his wife Sharon Cook (Love); his children, Oliver T. Cook, Jr. (Christina) of Kingston, NH and Jessica L. Cook of Beverly; his granddaughter Isabelle Cook of Kingston, NH.; and his sisters, Bonnie Hardy of Gloucester and Margaret Price of San Diego, CA. He is also survived by nieces and nephews.
