He was predeceased by: his parents, Richard H. Cusack and Sarah Cusack (Lyons); his sisters, Mary Perry and Katharine Morgan; and the mother of his children Virginia Gilligan.

He is survived by: the mother of his children Lucine Cusack; and his daughters, Angela Sullivan, Christine Kelly-Rose, Valerie Cardarelli (Jack) and Regina Cusack. He is also survived by seven grandchildren and two great grandchildren.
