She is survived by: her husband Donald Cummings; her children, Shannon Frias (Matthew) of Ashland, Daniel Tracy of Blackstone, Ashley Chipman (Robert) of Blackstone and Kelly Bellan (Ryan) of Bellingham; and her brothers, John Slade, David Slade and Steven Slade. She is also survived by many grandchildren and great grandchildren.
