She is survived by : her husband Saul Forman; her sons, Robert Seth Forman (Nancy Elrick-Forman) of Bedford, Richard Bradley Forman of North Attleboro and Gary Lyle Forman of Hyannis; her grandchildren, Richard Jr. of Attleboro, Jessica Forman-Kane (Kyle Kane) of North Attleboro and Alexander Scott Forman of Hyannis; her great grandchild Wyatt Kane of North Attleboro; and her brother Aristides Jr..
