She was predeceased by: her parents, Peter W. and Sophie C. Bardzik (Chwalek); her husband Robert J. Perlak; her brother Peter J. Bardzik; her grandson John Joseph Perlak; and her daughter-in-law Catherine Grivas Perlak.
She is survived by: her children, Mary P. Pajak (Stephen) of Wilbraham, Joseph R. Perlak and Anthony J. Perlak (Andrea); her granddaughter Stephanie Firely (Christopher); her great grandchildren, Logan and Hannah; her grandchildren, Matthew Pajak (Ana Mari), Samantha Pajak, Michael Perlak, Claire Perlak, James Perlak, Anna Perlak and Nathaniel Perlak; her brother John Bardzik (Kay) of Pelham; her niece Cathy Elko; her cousin Linda Ann Perlak; Teresa. She is also survived by many nieces and nephews.
In lieu of flowers, memorial contributions in Teresa's name may be made to St. Anthony of Padua Church.
