He was predeceased by: his parents, Walter Kozak and Helen Kozak (Tworig); and his sister Theresa Mille. He is survived by: his wife Denice M. Lapier; his children, John P. Kozak, II (Mariah Hill) of Dalton and Jacqueline Kozak of Hinsdale; his stepchildren, Rico Traversa of Peru and Heather Traversa (Jack Stimpson) of Pittsfield; his granddaughter Liliana Kozak; his stepgrandchildren, Rebecca Stimpson and Casey Wasille; and his siblings, Walter Kozak (Donna) of Cheshire and Nancy Kozak of Cheshire. He is also survived by many nieces and nephews.
Visitation will be held on Thursday, May 28th 2026 from 2:00 PM to 4:00 PM at the Paciorek Funeral Home (13 Hoosac St, Adams, MA 01220). A funeral service will be held on Thursday, May 28th 2026 at 4:00 PM at the same location.
The family requests the omission of flowers and suggests memorial donations be made to the Liliana Kozak Education Fund through the funeral home.
