He was predeceased by: his father Olivio Martin. He is survived by: his mother Judith Martin (DeCosta); his children, Tyler Martin and Avery Paige Sant-Martin; his brother Chris Martin; his godsons, Cory Pires and Grady Dion; his aunts and uncles, Ernie, Peggy Mendonca, Kenneth and Jackie DeCosta; his friend Bethany Turner; and his pet Meeko. He is also survived by many cousins and friends.

In lieu of flowers, donations may be made in Jeffrey's name to the American Heart Association, PO Box 417005 Boston, MA 02241.
