(Judy) and Claudette Forczyk, RN; her husband Robert; her grandchildren, Deannedre Dufour, Julienne Dufour (Jonathan Tully), Dominique Davis (Ryan), Klarysa Crape (Alex), Andrei Forczyk and Erik Forczyk; and her great grandchildren, Eustace, Esther and Euphemia.
