He was predeceased by: his parents, Juan Perez and Josefina Perez (Burgos). He is survived by: his wife Sandra M. Perez (Saster); his children, Eric Forrest (Kelli) and Cynthia Forrest of Franklin; his sisters, Lucy Perez, Chila Figuerora and Nilza Marte of Puerto Rico; his grandchildren, Laura Shaw, Ashely Donnelly, Eric Forrest, Jr. (Siiri), Dawna Forrest (Peter Santiago), Chris Forrest (Rebecca Boucher), Courtney Forrest (Gavin Williams) and Nicholas Forrest; his great grandchildren, Jimmy, Teagan, Chris, Jr., Magnolia and Deliah; and his sisters-in-law, Joyce Bardol (Bill) of Wrentham and Marena Papp of Franklin.
