He was predeceased by: his parents, Bernardo Urbina and Dolores Jimenez; his grandson Ardazan Jr.; and his siblings, Carmina Urbina, Jose Rafael, Sr., Bernardino and Luis Angel.

He is survived by: his wife Gloria J. Urbina; his daughters, Gloria M. Urbina (Moises Torres), Rosie Urbina and Jessica Dennis (Ardazan); his grandchildren, Tyree, Adonis, Eván, Serena, Anya and Tason; his niece Ana Ivette; and his nephew Jose Rafael, Jr..

In lieu of flowers, memorial contributions may be made to Saint Jerome Parish.
