He is survived by: his wife Jeanie Ptak (Stawniak); his children, Craig Ptak (Sarah) of Dalton, Andrea Ptak of Cape Coral, FL, Donna Hammer (Robert) of Great Barrington and Robin Kahler (Anthony) of Louisville, KY; his grandchildren, Kyle, Kyra Debkowski, Morgan, Brandon Hammer, Georgia, Greta Kahler, Aiden and Benjamin Ptak; and his great grandchildren, Emmalynn and Frankie. He is also survived by many cousins, nieces and nephews.
