He was predeceased by: his brothers, Joe, John, Leo and Bob. He is survived by: his wife Jacqueline Chase (Rust); and his daughters, Katelyn Chase (Kelley Chase) and Jacqulyn Greenleaf (John Thomas Greenleaf). He is also survived by many nieces, nephews, cousins, in laws and friends.
