She is survived by: her son Joseph MacKinnon of Walpole; her mother Jackie Jacobs (Maffei); her sisters, Lisa Jacobs of Danvers, Marcy Eastman of Mansfield, Kara Jacobs of Dedham and Rachel Jacobs of Randolph; her nieces, Diana and Jaclyn Eastman; her aunt Linda Maffei; her cousins, Marilynn, Jillian and Jessica Demaray; her stepmother Helene Jacobs of Raynham; and her close friend Ed Geller of Wareham.

In lieu of flowers, donations may be made in Jan's memory to the Massachusetts Humane Society or Make- A-Wish Massachusetts and Rhode Island.
