He was predeceased by: his parents, John Cislak and Christine Cislak; his brother Ben Cislak; his grandson Aiden Cislak; and his cousin Paul Cislak.
He is survived by: his wife Jacqueline Cislak (Blais); his children, Lisa Diotalevi (John), Tina Perkins (Brent), John Cislak and Tommy Cislak (Gina); his sister Phyllis Babineau; his grandchildren, Michael Diotalevi, Meghan Domingos, Danny Cislak, Christina Cislak, Gianna Szafranski and Sienna Cislak; and his great grandchildren, Addison, Emma, Blakely Meffen, Bohdi Domingos, Violet and Juduh Malonado. He is also survived by many loving extended family members, and dear friends.
In lieu of flowers, the family has requested that donations may be made in Edwards memory to Shriner's Hospital for Children, St. Jude's Children's Hospital, or Dakin Humane Society.
