Perlak of Ludlow, Massachusetts, born in Chicopee, Massachusetts, who passed away on May 31, 2023, at the age of 94, leaving to mourn family and friends. He was predeceased by: his parents, Joseph Perlak and Sophia Perlak (Rusin); and his wife Helen Perlak (Chwalek). He is survived by: his children, Edward Perlak (Patricia) of Dalton, MA, Frederick Perlak (Marita) of Chesterfield, MO and Regina Egan (Charles) of Brewster, MA.; and his grandchildren, Kimberly Perlak, Elizabeth Perlak, Ted Perlak, Daniel Perlak Anna McCarthy, Carolyn Egan and Matthew Egan. He is also survived by 6 great-grandchildren.
