He was predeceased by: his parents, Francis Kulakowski and Dorothea Kulakowski (Jones); and his brother Francis Kulakowski. He is survived by: his daughters, Leslie Kulakowski and Hilary Kulakowski (Kevin Dubovsky) of Kailua, HI; his brother Kevin Kulakowski (Kathy); his niece Kendra Myrmel (Nathaniel); and his nephew Kyle Kulakowski.
Visitation will be held on Tuesday, July 1st 2025 from 10:00 AM to 11:00 AM at the Our Lady of the Assumption School (40 Grove St, Lynnfield, MA 01940). A funeral Mass will be held on Tuesday, July 1st 2025 at 11:00 AM at the Our Lady of the Assumption Church (17 Grove St, Lynnfield, MA 01940). A burial will be held on Tuesday, July 1st 2025 at 12:30 PM at the Woodlawn Cemetery (302 Elm St, Everett, MA 02149).
In lieu of flowers, donations in David's name may be made to the American Stroke Association.
