He was predeceased by: his parents, Francis Kulakowski and Dorothea Kulakowski (Jones); and his brother Francis Kulakowski. He is survived by: his daughters, Leslie Kulakowski and Hilary Kulakowski (Kevin Dubovsky) of Kailua, HI; his brother Kevin Kulakowski (Kathy); and his sibling's children, Kendra Myrmel (Nathaniel) and Kyle Kulakowski.
In lieu of flowers, donations in David's name may be made to the American Stroke Association.
