She is survived by: her husband Jack Forman; her sisters, Beth Matheson of Acton, MA and Wendy Carpenter (Rustin Carpenter) of Acton, MA; her nieces, Kalya Smith (Peter Smith) of Portland, ME and Kristen Carpenter (Christian Rivera) of Encinitas, CA.; Dr. Rachel Forman (Boaz Avitall) of Milwaukee, WI and Fran Forman of La Jolla, CA..
