He was predeceased by: his parents, Adam Siudak and Katarzyna Siudak (Samek). He is survived by: his wife Helena Siudak (Surowaniec); his daughters, Joanna Weber, Michelle (Mathew Beauregard) and Priscilla (Igor Choma); his grandchildren, Cecilia Krazinski, Emily, Ashley Milaszewski, Abrielle, Eliana, Amelia Choma, Jacob and Joshua Beauregard; his great grandchildren, Eloise and Genevieve Krasinski; his siblings, Zbigniew Siudak of Poland, Josef Guzieczek and Janina Sarna; and his son-in-law Henry Weber. He is also survived by many nieces and nephews.
