He was predeceased by: his parents, Anthony Holak and Rita Holak (Endyke). He is survived by: his wife Maria Holak (Campusano); his children, Jennifer Hunter, David Holak, John Holak, Chris Holak and Joshua Holak; and his stepson Fraive Rosario Campusano. He is also survived by 5 grandchildren.
