He was predeceased by : his wife Lillian Garrity (Mahoney); and his sisters, Harriet Turco and Helena Dunlap. He is survived by : his daughters, Lillie Fournier (Robert) of Sarasota, Florida, June Fagan (Joseph) of Wilmington and Lorri McBride (David) of Andover; his grandchildren, Joseph Fagan III (Anna), Caroline Wilcox (Bryan), Elizabeth Kramer (Benjamin) and Laura Zawalich (Daniel); and his great grandchildren, Theodore, Charles, Violet, Kathleen, Madeline and Louise.

In lieu of flowers, donations may be made in Jack's name to the Relief Association of the Malden Fire Department, One Sprague Street, Malden, MA 02148.
