He was predeceased by: his parents, William Slade and Anne Slade (Ford). He is survived by: his wife Janice Marie Slade (Quinlan); his children, Frederick G. Slade Jr. (Lynne) of Sandwich, Richard W. Slade (Barbara) of Attleboro, Karen A. Michienzi (Thomas) of Attleboro and David M. Slade (Dawn) of Holliston; his grandchildren, Michael, Melissa, Amanda, Christine, Thomas III, Kevin, Nicole, Dylan and Daniel; and his great grandchildren, Alana, Evan, Caden, Quinn, Hadley and Maxwell. He is also survived by brothers and sisters and many nieces and nephews.
