He was predeceased by : his parents, Ralph B. Holbrook and Sally Holbrook (Calcari). He is survived by : his spouse Laurie (Lazelle); his sons, Anthony Holbrook Jr. of Orange, Thomas Holbrook of Erving and Michael Holbrook of Erving; his grandchildren, Angelica Holbrook and Trevor Holbrook; and his sister Karen Holbrook of Northampton.
