He was predeceased by: his parents, Thomas W. Cook Sr. and Margert Rita Cook (Burke); and his daughter Susan. He is survived by: his son Andrew Cook; and his sisters, Mary Jane Solimini and Barbara Sealund. He is also survived by many nieces and nephews.
