He was predeceased by: his parents, Manuel Souza and Angelina Souza (Dias); and his brothers, Richard Souza and Robert Souza. He is survived by: his close friend Diane I. Ondrick; his children, Bryan M. Souza of Warwick, Heather J. Souza of Warwick, David J. Ondrick, Jr. of Swansea and Denise Forget (Brian) of East Providence; his grandchildren, Raymond Ellinwood, Bryan Souza, Darrien Souza, Kevin Paiva, Nicole Andreozzi, Cody Ondrick and Steven Estrella; and his siblings, Beverly Almeida of Seekonk, Elizabeth Lawrence of East Providence, Joanne McCarron of Somerset and James Souza of Warren. He is also survived by several nieces and nephews.
