He was predeceased by: his father Richard Chase. He is survived by: his mother Julianna Chase; his wife Mary; his daughters, Kirsten, Kaitlyn and Keisha; his grandchildren, Elijah, Hailey and Xylo; his sister Wendy; his brother-in-law Tom; and his nieces and nephews, Casey, Carrie and Emma. He is also survived by Uncle, Aunts, cousins and many extended family and friends.
