He was predeceased by: his parents, Thomas P. Collins and Pat Collins (Nelson); and his siblings, Thomas (Kathleen), Candice and Brian.
He is survived by: his wife Maryann Collins (Nowak); his children, Jennifer M. Laverty (Randall) of Monson and John J. Collins Jr. (Melanie Gagnon) of Ludlow; his grandsons, Mathew and Adam Laverty; his brother Kevin (Mary) of Springfield; and his sister-in-law Kathleen of Springfield. He is also survived by nephews and nieces.
