He was predeceased by: his parents, Matthew Forte and Rose Forte (Mazzeo). He is survived by: his significant other Beate Bolen of Springfield, MA; his daughter Janet Forte of Brooklyn, NY; his sister Geraldine Hunter; his nephew Nicolas Hunter; and his former spouses, Inna Shames and Carole Forte. He is also survived by other family members, many friends.
