He is survived by: his wife Randi Cook of Lakeville; his daughters, Rachel Cook of Southington, CT and Rebekah Cook of Lakeville, MA; and his siblings, Douglas Cook (Denise) of Adrian, MI, Cathy Beam (Don) of Sheffield Lake, Bill H. Cook (Michele) of Adrian, MI, Sue Wilbar (Bob) of Adrian, Patty Cook of Adrian and Tim Cook (Amy) of Adrian.
In lieu of flowers, the family requests memorial donations be made to The Association for Frontotemporal Degeneration or the Silver City Baptist Church of Taunton, MA, designated for the Missions/Outreach fund.
